Patients with chronic conditions are among the costliest healthcare patients. Seventy one percent of all healthcare spending in the United States goes towards treating people with chronic conditions. It is also important to note that the prevalence of those living with chronic conditions increases with age – 80% of people 65 and older live with at least one chronic condition.
