Course description: 

More and more people are living with multiple chronic conditions such as diabetes, heart disease and/or depression. Patients with chronic conditions are among the costliest healthcare patients.  Seventy one percent of all healthcare spending in the United States goes towards treating people with chronic conditions. It is also important to note that the prevalence of those living with chronic conditions increases with age – 80% of people 65 and older live with at least one chronic condition. As the population ages and lives longer, the number of people with chronic conditions is expected to continue to grow. These patients may need support to help them maintain their disease processes outside of the hospital setting.
